Supply Lists & Back-to-School Info

Welcome to Meridian School!

We are so excited to have your student with us for this coming year. We look forward to a year of new experiences and achievements as we are led by our new Head of School, Dan Smith. In keeping with our green policy, we are directing you to our school website to download the documents you will need to complete and turn in. These documents are not only crucial to our being able to best serve your student in an emergency situation, but are required by law for us to keep on file for each family attending the school. Even if you have filled these out last year and nothing has changed, please take the time to fill them out completely and return them to the school as soon as possible.

Health Information Sheet: at the very bottom of the form, there is a section asking permission for office personnel to administer certain over-the-counter medications to your child. PLEASE make your wishes known about this procedure, as NO MEDICATION will be dispensed without express permission from a parent or guardian.
Field Trip Permission Form: all families must have a new signed form on file by the first day of school. This form will be good for all one-day field trip excursions.
Off-Campus Lunch Form: juniors and seniors in good academic and behavior standing may go off-campus for lunch if this form is signed by a parent and turned in.
Student Computer Internet Use Agreement: each student in grades 3-12 needs to complete this form and it must be signed by a parent. Please review with your child the proper behavior when using computers, internet and email.

Uniform Guidelines: Uniforms are purchased from Dennis Uniforms, Xpressions, the Meridian School store, or any other retailer. We also have many used uniforms available at the school for minimal cost. Click on the link and look over the guidelines then feel free to call the school with any questions.
Lower School Supply Lists: All lower school students should download their class supply list and purchase the items for the first day of class. In addition to these personal items, all lower school students will pay a $75 (K-3) or $80 (4-6) supply fee for purchasing group use items. Please pay this fee before the first day of class, checks payable to Meridian School.
Meridian Parent Organization: please take the time to look over the volunteer opportunities being offered and plan to sign up for a committee at Back-to-School Night. Meridian loves its volunteers! Also, please note that the $25 donation request is only per family, not per student. If you have questions, feel free to contact our MPO president, Jessica Salisbury.

Wellness/Illness Policies and Immunizations: if your child has a chronic illness or allergies, please be sure to contact our school nurse Kris Crowther kris.crowther@meridianschool.org. The following required immunizations should be completed before school starts and a copy of the student’s immunization record given to the school nurse on the first day of school:
  • 7th graders: Hepatitis A, Hepatitis B, Varicella, and TDAP Booster
  • Kindergarteners: 5 DTP, 4 Polio, 2 MMR, 3 Hepatitis B, 2 Hepatitis A, 1 Varicella (chickenpox) – history of disease is acceptable, parent must sign verification statement on school immunization record
  • PreK: DTP, Polio, Hepatitiz A, Hepatitis B, MMR, Hib, Varicella, and Pneumococcal – immunized appropriate for their age
Middle and Upper School Summer Reading List: All 7-12 grade students are require to read the books on this list before the first day of school.
Calendar Items:
  • Thursday, August 13 Open House 6-9pm – bring in your friends who may be interested in attending Meridian School
  • Thursday, August 27 Back-to-School Night 7-8:30pm – come meet your teachers and see your classroom
  • Monday, August 31 First Day of School 8:20 (bell rings at 8:15) – early morning seminary will begin this day also at Orem Seminary at 7:30am.
Middle and Upper School schedules and locker assignments will be handed out at Back-to-School Night. If you have any questions, feel free to call the school at (801) 374-5480. Summer office hours are Monday-Friday 9am – 12pm.
